When the GSRP enrollment form is submitted, I will give you a call so that we can set up a time for you and your child to visit my preschool and we can finalize your child's enrollment. 

 

A Health Appraisal Form and Shot Records are required.  A Health Appraisal Form is available at my preschool, feel free to drop by and grab a copy!

Please bring 3 documents to your enrollment appointment.

 

1) Your child's Birth certificate/Verification of Birth

2) Their shot record

3) The Completed Health Appraisal Form- Must be filled out by your child's Doctor

​

 

Also, keep in mind, your child must be 4 years old by December 1st to be eligible for enrollment.

 

There are limited spots, so don't wait!
